- The indications for surgery are pain, restriction and locking – and not joint noises
- Base management on a CLINICAL diagnosis supported by investigations (including MRI)
- Do not treat radiographs or MRIs, treat patients symptoms and signs
- MRI findings may be incorrect and outcomes do not correlate with MRI findings
- A stepwise approach to surgical intervention - conservative, arthroscopy, open surgery, alloplast
- Confirm your diagnosis with a joint LA injection
- Approach open surgery with an open mind - Base your intervention on the pathology found
- Outcomes in TMJ are: 80% get better - conservative, arthroscopy, open surgery
- There is no point in operating on anyone with a pain score <2/10 and mouth opening >30mm and no locking, you can’t guarantee improvement
- Alloplastic replacement has a good outcome in surgeons who are trained to use it and do so regularly
- Conclusions: First do no harm, second don’t repeat what has failed before, third don’t be frightened to ask for advice
