Abstract

Orthodontic relapse after combined orthodontic-surgical treatment has the potential to leave our patients unsatisfied and disappointed with their treatment. This lecture will explore the reasons for the relapse and different strategies that can be utilised to reduce the amount of unwanted post-treatment orthodontic changes.

We will explore the effects of dental relapse, skeletal relapse and long-term physiological changes on the alignment of teeth and occlusion. Within this we will consider the effects of periodontal factors, the quality of the final occlusion, impact of soft tissues and patients’ physiological adaptations to surgery, and finally growth, including the timing of surgical treatment and long-term age changes.

We will consider approaches to managing the problem, including the appropriate choice and timing of combined treatment, managing patients’ expectations, and consideration of different types of retainers. When considering the effectiveness of different types of retainers, we will briefly discuss the effectiveness of different types of retainers, survival rates, adverse effects and patient satisfaction with a variety of retainer types used after orthognathic treatment.
